Please bring the following to your appointment:

Offering Same Day Appointments!
Insurance Cards and State ID (Driver's License)
Please bring all insurance cards with you to your appointment. Your ID and cards will be scanned into our system. We ask to see your insurance cards at each visit. If you have Medicare, please make sure that we have your secondary insurance on file as well (If you have a secondary insurance). To learn more about Medigap, please click here: https://www.medicare.gov/supplement-other-insurance/index.html We do not accept straight Masshealth, but we do accept Masshealth that has Connector plans. Please go to the Connector website for more information: https://www.mahealthconnector.org/ We accept most major insurance companies. Call our office at 508-655-4422 if you have any questions.
Co-Payments and Deductibles
We are required by our participation in insurance plans to collect co-payments at time of service.  For your convenience we accept cash, checks and debit and credit cards. We will verify your co-payment when you call to book your appointment. Deductibles and co-insurance may change with your visit(s). If you have a question about your deductible or co-insurance please call your insurance company. If you have questions regarding your bill, please contact the number on your statement.  Our billing manager will be happy to help you.  
Referrals and Prior Authorizations
If your insurances require a referral, you must contact your PCP to obtain your referral as soon as you schedule your appointment with us. We will do our best to help you with both referrals and prior authorizations, but ultimately it is your responsibility to meet your requirements governed by your insurance company.
List of Medications
Please bring a current list of medications with you both over and under the counter. Please include any supplements and vitamins you take. A list of any allergies is important, too.
Prior Records
If you have records that are related to why you are coming in, please have your doctor send us these. You may have MRI scans, ultrasounds, CT Scans or blood work and lab tests. If you are unsure, please call us and ask, and we will be happy to help you.
ESWL Patients

Please remember that you must arrive 30 minutes early and to bring your medications with you to the office. Increase your water intake the day of your procedure and refrain from using any blood-thinning medications such as aspirin and ibuprofen. Please read the information sheet that your provider has given you.

Vasectomy Reversal Instructions

Pre-Operative

  1. Have a light breakfast the morning of the procedure. Do not drink coffee the morning of the procedure.
  2. Shave the scrotum and take a shower the morning of the procedure.
  3. Bring the bottle of xanax and Vicodin (hydrocodone) with you to the office. You will take both these medications in the office prior to the procedure.
  4. Bring an athletic supporter with you to the office
  5. Please arrive 15 min early to your appointment.
