Before your visit, please Log into the Patient Portal and ensure your information is up to date. This helps reduce paperwork at check-in and ensures more time with your provider.

- If you are new to our practice, we sent an email or text message with a link to set up your account.
Appointments
- You will receive a text message when an appointment is scheduled
- If you cancel or reschedule your appointment without providing least 2 business days’ notice, you will be charged a $75.00 late cancellation fee.
- If you do not attend a scheduled appointment, you will be charged a $75.00 no-show fee.
Please bring the following with you to your appointment:
- Your insurance card
- Photo identification
- Co-pay for the visit
- Any medical records pertaining to your appointment. Please ask your provider to fax to 508.655.9191
Insurance Referrals
- You are responsible for obtaining an insurance referral from your PCP if your insurance company requires one to see a specialist
- If your insurance denies the claim because a referral is not in place, you will be responsible for all charges.
- Please contact your Primary Care Provider office as soon as possible; this process can take up to two weeks

Location and directions:
We are located in the Medical Office Building adjacent to the Leonard More Hospital of MetroWest Medical Center.
- Enter the hospital complex via the Main Entrance (not the old Emergency Entrance)
- Immediately take the first right and go up the hill
- You will see a gray-blue building which is the Medical Office Building
- Free parking is available in the lot
- Once you enter the building, go to the third floor
- Our office is at the end of the long hallway
